The Hoosier Youth ChalleNGe Academy intervenes in the lives of at-risk youth by training, mentoring, and developing these individuals into responsible and productive citizens while in a controlled, quasi-military, residential setting, followed by a post-residential phase.

Cadet Application

Now accepting Cadet Applications.
Applications are accepted all year on a first come first serve basis.

Please submit your application as early as possible.

If you don't have the time to download a full application packet please provide us with some quick information through our Contact Us page and our Regional Recruiting Assistant will contact you.

 

Download Cadet Application
In order to apply you have to download and complete the application in its entirety and submit it either by mail or by delivering it to a Regional Recruiting Assistant at a seminar in your area. In the event that you cannot download the application below, contact the Regional Recruiting Assistant in your area to request one be mailed to.

Please completely fill out the Cadet Application (PDF)*. Please answer all questions. If you answer yes to any of the medical questions that may have occurred within the last 5 years you must explain and provide a statement of release from your doctor permitting you to attend the program. If you currently have not received a withdrawal form provided from the last school you attended.

Mentor Application (PDF)* - A Cadet Application isn't complete without this paperwork for two Mentors (Primary and an Alternate Mentor Nominee). The Mentor will be one of the most important members of your team so make sure you understand what the Hoosier Youth ChalleNGe Academy's mentoring role is about and choose your nominee(s) wisely! Click on this link to get  information on the role for a mentor.  Your potential Mentor applicants must complete this application, be able to pass a background history check, and be willing to attend a three-hour Mentor training prior to the deadline.
