Required Documents

Following are documents that are required to complete your application:

• NEW - Sports Physical required after applicant has been interviewed/accepted into the program.  The Physical Exam Form (PDF) should be filled out by a physician and turned in to the Academy before applicant will be allowed to in-process.

• Birth Certificate - We require a copy of the "state" (not the hospital) birth certificate.  If you cannot locate your copy, please refer to the Vital Records Dept. of the Oklahoma State Dept. of Health to obtain a certified copy of a birth certificate or call (918) 594-4840 - Tulsa area; (405) 271-4040 -OKC area.
  
  
• Social Security Card - We require a copy of a signed card.  Many times the card will be obtained when the child is still an infant and families do not remember to have their child sign the card when they get older.  If you do not have one or have lost the applicants card, please Contact the Social Security office in your area to apply for a card or request a duplicate card.
   
• Drivers License - Need a copy if applicable.
   
• School Transcript - At this time, the transcript does not have to be an official one...just a copy will do.
  
  
• Shot Records - Each applicant must have the following immunizations:
  • MMR#2 (measles, mumps and rubella),
  • PPD (TB skin test within the last 6 months or current),
  • TD (tetanus within the last 7 years),
  • Meningitis shot
    
    
• Health Insurance Card/CDIB - Must have a copy of both sides of these cards if applicable.  Not having insurance coverage will not keep you from being accepted into TYA.
   
• Certificate of Degree of Indian Blood (CDIB) - if applicable
